Description: This is a point-of-care manual with essential information on managing patients with pulmonary diseases as well as medically critically ill patients. The book is a collection of decades of teaching wisdom from Dr. Landsberg that incorporates unique factoids and algorithms that are rarely found in other textbooks. The accompanying website provides access to all the chapters and images in a well-known format.
Purpose: According to the preface, this book represents teaching materials and illustrative cases that the author has compiled and refined over more than a decade. It incorporates diagnostic and therapeutic algorithms for common diseases encountered in the ICU and in pulmonary medicine, addresses common misconceptions, discusses basic pathophysiology, and provides illustrative cases to help get the message across. These are worthy objectives, and the book does meet most of them by providing a concise, portable, yet comprehensive review of topics that are difficult to grasp and fraught with misconception in this field.
Audience: The book is written primarily for physicians in training, including residents and fellows in pulmonary and critical care medicine. However, it should be equally useful to medical students and as a refresher to practicing pulmonologists, intensivists, and internists who take care of patients with respiratory ailments and critical illness. Dr. Landsberg has taught and practiced the specialty for decades in a premier American institution.
Features: The book is divided into pulmonary and critical care sections. The pulmonary section covers the approach to hypoxemia, hypercapnia, ventilatory failure, pulmonary function testing, chest imaging, dyspnea, heart failure, pulmonary hypertension, obesity hypoventilation, lung cancer, aspergillus lung disease, cough, sarcoidosis, venous thromboembolic disease, pneumothorax and parenchymal lung disease. The critical care portion covers shock, noninvasive and invasive mechanical ventilation, hemoptysis, electrolyte regulation, altered mental status, diabetic emergencies, advanced care planning, and code discussion in the ICU. It also has other high-yield topics like bronchiectasis, pneumonia, pleural effusion, etc. The unique features of this book are the concise presentation, bulleted format, discussion of common misconceptions, ample diagnostic and management algorithms, common questions on the topic from learners, and case vignettes. Rarely found in other books, these features really magnify the impact of the book on learners. A few shortcomings include poorly formatted pictures from patients’ charts and the extremely small font used in certain pictures and algorithms. Even though several topics in pulmonary and critical care medicine are missing, the book doesn’t set out to be a comprehensive textbook on the subject and hence remains immensely valuable.
Assessment: This point-of-care, portable manual on the most common problems encountered in pulmonary and critical care medicine is intended to enforce certain key concepts and clear up common misconceptions, not be a comprehensive textbook. This is one of the most unique and high-yield writing on this topic that I have come across, by an author who is obviously a very astute clinician and an expert in his field. I highly recommend this book to every internal medicine resident and pulmonary/critical care fellow in training.
